ACCEPTANCE OF WORK By signing or digitally approving this Estimate / Invoice you agree that the prices, specifications, and conditions are satisfactory on this Estimate / Invoice and are hereby accepted. You are authorizing Mold Zero to perfor the work as specified. Payment will be made as outlined on this Estimate / Invoice. A 50% deposit is required to schedule the job and the remaining balance is to be paid on the day of treatment prior to commencing with the service. Deposits are non-refundable unless Mold Zero is unable to perform the work specified.
SELF PAY / INSURANCE: If the client decides to self-pay with the intent of asking for reimbursement from their insurance company, then throughout the entire process the client is responsible for all payments for the work. If client decides to go the insurance route right from the beginning, a $1000 deposit is required to schedule the job. This amount goes towards the invoice amount. If the insurance pays the bill in full, the deposit will get refunded to the client. If not, Mold Zero keeps the deposit.
MOLD ZERO’S TERMS AND CONDITIONS Estimates are valid for 30 days. If a deposit has not been taken, the estimate can be updated, cancelled or revoked at any time without any explanation necessary. Final Payment is collected the final day of the fogging.
CUSTOMER RESPONSIBILITY STEPS FOR DAY OF THE FOG The property must be vacant of all humans and pets during the fogging treatment. The technician will inform you when the fogging treatment is complete. Please note that individual tolerances will vary. Some people are more sensitive to the odor of vinegar than others. We recommend at least two to three hours before re-entry. If the smell is still very strong for you, then begin to air out the house, or turn the AC back on to help dissipate the smell. Only you can be the judge of your comfort. The original source of dampness (water leak, roof leak, flooding, high moisture, etc.) must be completely resolved prior to treatment. If not resolved, Mold Zero is not liable for any mold growing back. We recommend that all cabinet doors, drawers and dishwashers be open to allow the fog to more easily penetrate those spaces. We ask that the Air Conditioner is set on cool the night before we arrive for the treatment and at least below 74 degrees. If the property is equipped with a fire detection system that reports to a central monitoring service, that service must be informed not to dispatch the fire department during the fogging treatment. Smoke alarms should be set to the “test” setting wherever possible. Battery-operated smoke alarms should be taken down, or have their batteries removed. We ask that all photographs that are exposed to the air be removed. Any photographs in albums or behind glass are okay to leave as is. This does not apply to paintings. Any sterling silver or brass that is exposed to the fog may tarnish. Please put away jewelry, picture frames, candle holders, knives & forks, etc, that are made of these metals.
ALL OTHER DAYS If there is plastic containment up in the home due to removal of contaminated material, we ask clients to not go in and out of the containment or remove it without consulting with us first. Containment is used to prevent cross contamination during mold remediation. If there are air scrubbers, fans, dehumidifiers or other various equipment placed in various areas of the home, under no circumstance should you turn them off without consulting with us first. We are aware the machines are noisy but they are there for a reason. Turning off the machines will extend the length of time it takes to complete our work and it will reduce the effectiveness of the treatment. If you were to turn them off incorrectly, it would likely result in cross contamination. When the removal of kitchen or bathroom counter-tops is required, every effort will be made to salvage the counter-tops. Mold Zero, however, is not liable for accidental breakages that may occur. Additional costs may occur or warranty could be voided if certain prep steps are not followed and result in expanded scope of work due to excess time.
REATMENT OVERVIEW: The first dry fog application is our InstaPURE product which is certified by the US Government as a sterilant which will eliminate mold, bacteria, and viruses. The second dry fog application is our EverPURE product, which creates a microscopic barrier on all hard surfaces and prevents future mold, bacteria and viral growth from occurring. This can last up to a year in low traffic areas (Residential Homes). In high traffic areas (Commercial Buildings) it is approved by the EPA to work for up to ninety days after application. EverPURE has the ability to remove odors but we do not guarantee that all odors will be eliminated. Due to state regulations, Mold Zero is not allowed to clean any mold inside the HVAC system, beyond the filter. The treatment is non-toxic and safe. The amount of time required to complete the application will depend on the size of the space involved. You can return to the property two to three hours after treatment is completed. There may be a slight vinegar-like smell, which will dissipate over the next 24 to 48 hours. All fabrics, dishes, bedding, etc. are safe to use after our treatment with no additional cleaning necessary.
POST REMEDIATION VERIFICATION: Mold Zero determines the effectiveness of a remediation project by doing an Air Quality Sampling of the space and a control sample outside. If the results indicate that the levels of mold are the same or less then the control sample, the remediation project is considered to be effective and at normal ecology. If the samples do not pass per the agreed upon scope of work, Mold Zero will come back to treat again.
POST TESTING: Mold Zero will share the final test result that will support the completion of the project with only the client. Any intermediate tests are at the discretion of Mold Zero. They will not be charged to the client and will be kept strictly for Mold Zero personnel for consultation purposes, for tracking progress, and treatment efficacy. These intermediate tests will be communicated only to Mold Zero staff and any IAQ specialist involved in the project.
PROTOCOL COMPLIANCE Mold Zero standardly follows and complies with NORMI protocol. We also follow the protocol laid out by a 3rd party company.
TREATMENT DOES NOT INCLUDE: Finding and handling the original source of dampness (water leak, roof leak, etc.) (This action should be conducted by a qualified leak specialist.) Repair and rebuild of any damage created by the mold. (Mold stains can exist after mold has been removed and Mold Zero is not responsible for it unless specified in the scope of work).
PROVISIONS / CANCELLATIONS: All Contracts must be signed prior to scheduling the remediation job. Mold Zero locks in your date once the deposit has been given and does not take any other jobs. If a job needs to be rescheduled for any reason, 72 hours advanced notice must be given to Mold Zero in writing or client may be subject to a $200.00 change order fee. If a job needs to be canceled for any reason, 72 hours advanced notice must be given to Mold Zero in writing or 20% of the original deposit will be retained as an inconvenience fee. A 3% merchant credit card fee will be retained for all refunds. Any and all refunds for deposits or completed jobs will automatically disqualify one from further services or treatment from Mold Zero. Specific situations will be reviewed upon request. CHANGE ORDERS If the scope of work changes due to new discovery after remediation has commenced, the remediation project will be suspended until the required contractual adjustments are made and approved by the client. If the work site is found to be dangerous or hazardous, the work will not be performed and the contract will be canceled.
WARRANTY: Residential: Mold Zero services are guaranteed for a period of 1 Year from the treatment date when the entire home is treated. If at any time during the following 1 year mold is found in the area of service at a greater density than the outside air, Mold Zero will re-treat the home at no additional charge. This includes fogging and cleaning the home. It does not include additional tear out.
WARRANTY IS VOID if additional water is introduced into the premise, (i.e. groundwater, broken pipe, appliance leak, roof leak, high indoor humidity levels, etc.) or if the original source of dampness was not handled prior to doing the treatment. High humidity is anything over 61%. Mold can start to grow at that amount. Any new mold situation is not covered under the warranty. Removal of the non-viable mold is not covered under the warranty. Any additional testing requested is the responsibility of the homeowner. We do not offer free testing. We do honor any 3rd party testing. We will not observe petri dishes as relevant sources of information before or after we have treated a home when determining where a potential mold problem exists or has been handled. The definition of petri dish: “a shallow, circular, transparent dish with a flat lid, used for the culture of microorganisms.” They are essentially made for the purpose of culturing (growing, nurturing) microorganisms (which include mold spores). Only samples from certified laboratories will be used and or referred to in these scenarios. When we send our quality control specialist, any recommendations given to you should be followed as this will prevent the mold from coming back. Commercial: Mold Services guaranteed for a period of 3 months from the treatment date when the entire building is treated. If at any time during the following 3 months, mold is found in the area of service at a greater density than the outside air, Mold Zero will re-treat the building at no charge.
